Provides one-stop access to a wide range of palliative care support services within Vancouver,
Palliative care support services includes nursing care, home support/personal care supports, care management, rehab therapies, home hospice specialist service, access to residential care hospices or acute care palliative care units.

Referrals are accepted from any source including the public, family doctors, BC Cancer Agency, outpatient units, and acute care hospitals outside of the Lower Mainland.
