Health Services for Community Living - G.F. Strong Rehabilitation Centre

Provided by Vancouver Coastal Health

Provides rehabilitation services to adults with developmental disabilities.
Provides non-emergency nursing and rehabilitation services to adults with developmental disabilities living in the community. Ranges from short-term clinical intervention to end-of-life care and support. Facilitates access to a range of community health services, including physiotherapy, occupational therapy, nursing, nutrition and dental hygiene.

The team also provides individual and group education on self-care management for clients managing chronic illness. Staff members focus on training and supporting family members and caregivers, providing referrals to healthcare providers, advocating for clients needing specialist care, and working with staff, families and clients to create and implement care plans for specific health concerns.

Referral Details: Referrals must be made by a physician or nurse practitioner.
